Deciding on moving into residential aged care? This can be a difficult decision to make, especially if you are already in hospital or struggling at home. At Francis of Assisi Home, we will endeavour to make this process as smooth as possible, explaining all steps clearly. There are a few basic steps that you will need to take to prepare for residential care:

1. Register with My Aged Care.
2. Request an ACAT assessment for either Respite or Permanent residential care.
3. Make an appointment with Services Australia to do an Income and Asset Assessment. Should you choose not to complete the assessment, you may be asked to pay the maximum amount.
4. Fill in the Application for Entry form below or visit our Home to request a paper version.
5. You can also book a tour of the facility. Most tours are scheduled for Tuesday or Thursday afternoons.
